New Patient Information
Welcome to the office of Trent C. Holmberg, MD. We make it a priority to provide quality care with a friendly atmosphere. Please print and completely fill out ALL new patient forms. You must bring these to your first appointment or you will need to reschedule. Forms are usually emailed to you, but can also be downloaded by clicking here or on the "New Patient Forms" tab above. If you do not have a printer, you may also fill them out in the office by arriving no later than 45 minutes early. If you appointment is a virtual appointment, please email or fax your forms back no later than two days before your appointment.
If reasonably obtained, please have any previous mental health records faxed to our office before your appointment. Also, please bring a complete list of current medications, dose and directions.
New patient evaluations generally last one hour. Cancellations must be made at least 24 hours in advance to avoid a cancellation fee. If we are not in the office you may leave a message. Please see our payment policy for new patient cancellation or missed appointment fees.
Payment in full is due at the time of service. Unless otherwise notified, Dr. Holmberg is not a "provider" for any insurance plan. However, we do provide you with an invoice that contains the necessary information for insurance reimbursement. It will be the patient's responsibility to check with their insurance company regarding their reimbursement policy. For an updated clinical fee schedule, please call the office.
Medicare Patients! Dr. Holmberg is a Medicare Opt-Out or Non-Provider, which means that if you are on Medicare and choose to see Dr. Holmberg you are required to sign a contract stating that you or Dr. Holmberg, will NOT submit to Medicare for any type of reimbursement. This is Medicare's policy when you see an Opt-Out Provider.
