Patient Information
As of August 1, 2022, Dr. Holmberg transferred his patient practice to Strive Psychiatry located at 11762 State Street, Suite #360 in Draper, Utah where he has also been providing TMS treatments since March of 2021.
Fees and Insurance. While Dr. Holmberg is contracted with insurance companies for TMS treatments, his patient practice will remain self-pay. Please contact Strive at 801-494-1922 for a current fee schedule.
Appointments
Dr. Holmberg will be seeing patients at Strive Psychiatry on Wednesdays. To make an appointment please call Strive at 801-494-1922 option 2. Dr. Holmberg will offer 15- or 30-minute appointments only due to his limited schedule.
Records
To have your records sent to Strive Psychiatry or another provider you must complete and sign the AUTHORIZATION TO USE AND DISCLOSE PERSONAL HEALTH INFORMATION (click on this link). This form can also be found in the forms tab or by calling 801-432-2077 or by sending an email to tchmdpc@gmail.com. The completed form can be mailed, emailed to tchmdpc@gmail.com or faxed to 801-432-2079. Please send this form back as soon as possible to allow adequate time to transfer your records.
Medication Refills
If you are changing providers, please make an appointment with your new provider as soon as possible to ensure continuity with your medication(s). Dr. Holmberg will provide medication management through this office for up to 60 days after August 1, 2022 or until your first appointment with your new provider, whichever comes first.
